Chemical Safety Compliance Failures Lead to $794,000 in OSHA Penalties
Chemical safety compliance failures at a Maine pulp mill have resulted in more than $794,000 in proposed OSHA penalties after two workers died from exposure to hydrogen sulfide gas during a cold weather shutdown. Federal investigators also examined a separate chemical fire at the same site weeks later, caused when thousands of gallons of hydrogen peroxide mixed with accumulated wood pulp. The employer received citations for missing gas alarms and breathing devices, along with gaps in worker training, respirator fit testing and medical evaluations.
The case is notable for its history. OSHA records show the company had been inspected thirteen times since 2017, including one earlier incident in the same area that sent fourteen employees to hospital. Repeat findings like this point to a training and verification system that was not keeping pace with known hazards.
FlexManager’s Training Manager and Chemical Library help close exactly this kind of gap, keeping hazard specific training current and tied to the substances actually in use on site, while Risk Assessments and Incident Reporting create a clear record connecting past findings to the corrective action taken.
If an inspector asked today, could you show that every worker handling hazardous chemicals on your site has current, verifiable training? Book a demo with FlexManager to see how our Training Manager and Chemical Library support that record.

Why do Incidents Happen at the End of a Shift?
In many industries, patterns in safety incidents are rarely random. One trend that consistently emerges in HSEQ reporting is a spike in incidents toward the end of a shift. For safety and operations professionals, understanding why this happens is critical to improving compliance, reducing risk, and protecting both people and assets.
