U.S. Paper Mill Cited over Employee Safety

A tank valve accidentally left open

Washington’s Department of Labor & Industries (L&I) has cited Nippon Dynawave for endangering workers. It arrives after a worker at its Longview paper mill fell into a sinkhole filled with hot pulp. The incident predates a separate tank failure that killed 11 workers there in May.

So, what happened? A tank valve was accidentally left open, spilling gallons of 145°F pulp per minute for two hours. An employee waded in to close the valve, then fell into a sinkhole the spill had created, narrowly escaping.

The company was cited for failing to protect workers from extremely hot materials and engulfment hazards. Additionally, its pulp system was missing from a required hazardous materials piping safety program.

Previously, L&I fined the mill $2,890 after managers could not produce records of required inspections on an ammonia tank. Neither citation relates to the May 26 tank failure, which remains under investigation. This latest citation could come with a fine of $77,000, although Nippon Dynawave has 15 days from the date the citation was issued to appeal.

For businesses running high-risk processes, the lesson is clear: known hazards need controls, not workarounds, and every inspection needs a paper trail.
