In San Antonio, a building contractor and the staffing company that supplied its worker were both cited following a fatal incident involving a mini-excavator in a crawl space beneath an elementary school.
Both cases share a common thread. Confined space work and labour-hire arrangements depend on clearly shared safety responsibility, and both are areas where documentation gaps go unnoticed until an incident forces a regulator’s hand.
